For a new patient appointment with one of our specialists, please call 203-785-4081. For follow-up appointments or other questions, please call us toll free at 800-395-7949. We are open Monday through Friday, 8:00AM-4:30PM, except during major holidays. We welcome the opportunity to provide second opinions, especially related to surgical correction of simple and complex strabismus problems in children and adults.

Before scheduling your appointment, please ask your referring physician to fax the results of your biopsy to 203-785-5256. Most of our referring physicians are familiar with our policy and automatically fax your results. Once the biopsy results have been faxed, please call our office at 203-785-3466 to schedule an appointment with one of our physicians.

For an appointment with one of our specialists, please call 203-785-4081. For other questions, please call 203-785-2022. The parking lot at 1 Long Wharf is currently undergoing a resurfacing project. In the interim, there will be designated spaces for patient parking, along with access routes to the building. Parking lot attendants will also be available for assistance, if necessary. Please allow an extra 10 to 15 minutes.

To schedule an appointment or make a referral, please call 203-785-4085 and provide the following information: Demographics of the patient Insurance information Details about the nature of the problem Any notes, reports or imaging results This information can be easily filled out in our referral forms: Outpatient clinic visit EMG/nerve conduction study Once we receive the referral we will contact the patient to schedule an appointment. After we see the patient we will send a full detailed note regarding the visit will be faxed to the referring provider.
